Appointments

SCHEDULING AN APPOINTMENT

We invite you to call our office at 419-238-1000 to schedule an appointment with any of our trusted counselors. If you prefer, we can schedule your appointment through email at melanie@counselingofficevw.com.  Services are provided on an appointment basis. If you call our office and no one is available to take your call, we encourage you to leave a voice message and one of our staff will return your call as soon as possible.

FEES

All counselor fees are based on a $120.00 per hour rate and billed in 15-minute increments. Fees cover one hour of clinical service, which consists of 50 minutes of direct service to the client and 10 minutes of record keeping and review. Payment for service is required at the time of service. For your convenience, we accept cash, checks and major credit cards. Any checks returned for non-sufficient funds will incur an additional $30.00 charge. We expect payment for the full amount at the end of each session.

Indirect services provided to coordinate your care – such as email correspondence, meetings and letters to other professionals or institutions – will be billed at the rate of $25.00 for each 15-minutes spent.

INSURANCE

In many instances, insurance benefits may be available for reimbursing a portion of the client’s fees for counseling services. As a client, you are responsible for filing any claim with your respective insurance company for applicable reimbursement. Van Wert Counseling Services will provide clients with all necessary information in order to file for reimbursement. You will also be responsible for any notification or preauthorization requirements that may be applicable to receive benefits. We suggest you contact your insurance carrier before your appointment regarding your coverage. Your carrier will need to know that we are an out-of-network outpatient mental health counselor.
